To determine one's last name for Master Agreements, take the Driver's License and use the last name found (see LINE 1), even if multiple last names are found for the last name field.
For example, Mary Joan Rivers Doe would be written LAST NAME, FIRST MIDDLE NAME as Rivers Doe, Mary Joan. Do not use a hyphen unless one is given on the license itself.
